Membership Agreement and Illinois Consumer Protection

Membership Terms:

◉ Annual membership fees as disclosed in the membership agreement

◉ Services included and excluded are clearly defined

◉ Right to cancel membership with appropriate notice

◉ Refund policy in compliance with the Illinois Consumer Protection Act

◉ No discrimination in membership acceptance (Illinois Human Rights Act)

◉ Controlled substances: Prescribed in accordance with the Illinois Controlled Substances Act

◉ Specialty referrals to Illinois-licensed specialists when appropriate

Telemedicine Compliance – Illinois Requirements

Illinois Telemedicine Act Compliance:

◉ Telemedicine services are provided only to established patients or after proper patient evaluation

◉ Technology meets appropriate standards for clinical decision-making

◉ Patient identity verification protocols

◉ Informed consent for telemedicine services

◉ Documentation standards equivalent to in-person care

Technical Requirements:

◉ Secure, HIPAA-compliant platforms only

◉ Patient is responsible for a reliable internet connection

◉ Audio and video quality are sufficient for medical assessment

◉ Alternative communication methods available if technology fails

◉ We reserve the right to terminate website access for violations of the terms

◉ Patient portal access continues until formal termination of the care relationship

◉ Data backup and transfer protocols clearly defined

Updates and Changes

Terms Modification:
